​Version 11 April, 2021 Participant Information Sheet Thank you for expressing interest in participating in this research project, designed to collect information about how you (or your child) feel after osteopathic treatment. What will I have to do? You will be asked to complete a questionnaire using the Internet or an app that you can access on your smart phone, tablet computer or iPad. This will happen once at your (or your child’s) first appointment or very close to the start of treatment. You will also be asked to complete a further three questionnaires, one week after treatment, another at six weeks, and a final one three months after the first treatment. Each should take no longer than five minutes to complete. This information will be analysed by researchers from the National Council for Osteopathic Research (NCOR)*. We hope the study will help to improve osteopathic care. To access the questionnaire, please type the following link into your browser window http://www.clinvivo.com/ncor/ When you reach the site please click on one of the images from the list below to complete the questionnaire. The app can be downloaded easily from the app store or Google play by searching for “Clinvivo”. If you download the app to your device, please check in your “settings” to ensure you allow notifications to your phone. If you prefer, you can use the QR code to access the app for your smartphone, tablet, or iPad. Your data: The information we collect will not include your name, address, or date of birth and your information will not be made available to anyone else, not even your osteopath. They will receive a summary report periodically about outcomes of care at their practice. We will not sell or use your data for any commercial purposes. You should only agree to take part if you want to; it is entirely up to you. If you decide to take part, your completion of the questionnaire will be taken as your consent to participate. You are still free to request that any data you submit be withdrawn at any time, and without giving a reason. Your data will be stored for 10 years by the National Council for Osteopathic Research. If you complete the questionnaire you can request withdrawal of your data at a later time by contacting Carol Fawkes ([email protected]). If you choose not to take part there won’t be any disadvantages for you and you will hear no more about it. Choosing not to take part will not affect your access to treatment or services in any way. It is important for us to make you aware that there is an extremely low risk that your phone could be hacked if you decide to use the smartphone app. If this happened the information you provide in the questionnaire could be viewed. This could happen if your phone is stolen, for example.
